- Check for TMJ or bite issues
- Review your overall dental and medical history

What to Bring
To ensure your visit goes smoothly, please come prepared with:
- Completed patient forms (forms must be submitted before we can schedule your appointment)
- A list of any medications you are currently taking (or bring a written list we can copy if you are unable to complete this section online)
